Most case management service providers monitor their cases by phone or in person. At Med NY we deliver an integrated service model, combining both field and telephone-based case management services. This unique combination helps maximize outcomes and minimize expense.

Field Case Management

Med NY’s field case managers are strategically located across New York State, to provide expert, on-site service at the local level.

  • Prompt and personal contact with the injured worker, his/her healthcare providers, and the employer-within 24 hours of referral-ensures that all parties are working together to develop and expedite a plan of action that helps promote a prompt return to work.
  • Our certified case managers are trained to provide a full spectrum of field-based services – from abbreviated task assignments to full-scale, catastophic case management.

Telephonic Case Management

Our extensive telephonic case management team, centrally located at our corporate office in Syracuse, NY, work alone or as a team with our field-based case managers.

  • Centralized intake of claims provides the benefits of early intervention by our telephonic team. Initial contact is made within 24 hours.

  • Following the same three-point contact as our field-based managers, our telephonic case managers develop a plan of care and manage access to our extensive medical support network.  
  • Where field-based intervention can improve patient outcomes, our telephonic team will refer assignments to our field-based team.

Return-to-Work Programs

A complete return to safe, gainful employment is the ultimate goal of our case management services.

Med NY Case Management can help you create, implement, and monitor a customized return-to-work program to meet your specific needs. Our case managers are trained to focus on return-to-work from the initial assessment to final case closure.

Med NY works closely with the employer to explore realistic options for a timely and safe return to work.  Light duty, modified duty, and temporary transitional work options are discussed. Worksite evaluations are performed and accurate job descriptions are developed. The attending physician is educated as to the physical requirements of the job, to ensure a safe and appropriate return to work.

Vocational Services

If the injured worker cannot return to work with the employer of injury, then Med NY is able to provide assistance in vocational rehabilitation and job placement services.  Certified, master’s level vocational counselors are utilized to perform vocational assessments, transferable skills analyses, plan development, job readiness coaching, and direct job placement assistance.  Our goal is to assist the injured worker in a timely and safe return to gainful employment.

Med NY is also able to provide Hypothetical Vocational Evaluations, to assess an individual’s level of employability.  Whether it be an abbreviated vocational assignment, or full-scale vocational rehabilitation assistance, Med NY has the range and flexibility to meet your needs.  We utilize the telephone and the Internet as fully as possible to maximize efficiency & minimize expense.  Our state-of-the-art technology — Xclaim — is an Internet based, paperless system, and offers secure interfacing to our clients.

Utilization Review

Med NY Case Management can provide a complement of utilization review services including pre certification, concurrent, and retrospective reviews.

  • A Medical Review Board compares requested treatment plans to national practice standards for medical necessity, treatment setting, and frequency and duration of care to ensure that appropriate treatment is being recommended.

Safety Education and Ergonomic Evaluations

Med NY has a staff of qualified safety and loss control and certified engineering specialists that will work with employers to improve workplace safety and employees' interaction with their work space.
